We need specific information to be able to consider whether or not you might be eligible for this government program. Having a medical diagnosis or disability does not automatically qualify you for a Disability Tax Credit. The requirements of this program are much more stringent, and whether or not you qualify is decided by government adjudicators based on the information submitted. Qualifying for a Disability Tax Credit requires there to be a very significant functional impact on your basic activities of daily living.

The above link will take you to a short Government of Canada questionnaire that will help you determine if you should submit a Disability Tax Credit application.
If, after completing the above questionnaire, you believe that you might be eligible for the Disability Tax Credit, print the screening form (below) and return it to our office as this is the information your physician will use, along with your chart, to complete the application. Occasionally, an appointment will be required. The more details you can provide about the impact on your function, the better chance of success you will have in your application. Please provide specific examples.
Completion of the Disability Tax Credit application is not a covered service under OHIP and there is a fee of $150. Payment is required in advance and can be made in person, over the phone, or online using the "make a payment" option. Anticipate completion within 6 weeks.
