Prescription refills provided at an appointment will continue to be, as always, free of charge. Refills provided without an appointment (eg: requests received by fax from your pharmacy) will be subject to a convenience fee (currently $20, but subject to change). In order to avoid this fee, please book an appointment prior to running out of medications. After making your payment, please contact our office via message or phone call to request your refill.
We implemented a no show/late fee policy in October 2017. This policy was created due to the large number of patients missing appointments, which decreased access for other patients. Missed appointments are not covered by your health insurance, OHIP.
When canceling an appointment with a family physician or nurse practitioner, we require 24 hours' notice. When canceling an appointment with a nurse or specialist physician, we require 2 business days' notice. Please note that patients arriving 8 minutes after their scheduled appointment time will be considered having missed their appointment.
Letters can be written to help you get coverage for physiotherapy, registered massage therapy, chiropractic care, custom orthotics, compression stockings, etc. As these letters are for insurance coverage rather than to obtain the services, they are not covered by OHIP, and are subject to a $20 fee.
Sick notes should not be required by your employer. However, if you would like to request a sick note, we will write one for you. You need to provide us with information regarding your illness in order for us to write a letter. We may require you to have an appointment in order for the letter to be written.
Sick forms should only be required as a proof of illness for short or long-term disability. We will complete them for you, and fax them to Occupational Health or your insurance company upon your request. Please provide us with information regarding your illness in order for us to complete the form. We may require you to have an appointment in order for the form to be completed.
These forms take a considerable amount of time to review your chart, and provide you with the best chance of qualifying for this tax credit. We have created a form for you to complete in order to help us with the form. In addition, the Government of Canada often contacts us for additional information. The fee includes completion of the supplemental form(s). Please contact our office to request a copy of the information form. Upon completion, we will send your completed application to the Government of Canada Tax Office.
