Please note that these roster forms and intake packages are only for patients who have been contacted and instructed to complete them. People who are not current patients or who have not been contacted by our office to complete forms will NOT be accepted as patients by submitting the above forms.
All forms should be returned to our office via the Pomelo (HealthMyself) online portal (ideally) or dropped off in person to our office.
IF YOU DO NOT COMPLETE YOUR NEW PATIENT INTAKE FORM (like below) AND RETURN YOUR ROSTER FORM AT LEAST 7 DAYS PRIOR TO YOUR INTAKE APPOINTMENT, YOUR APPOINTMENT WILL BE CANCELED.
